Dion’s Chicago Dream is committed to advancing health equity and food security in Cook County through its innovative Produce Prescription Program (PPP), which leverages Medicaid to deliver fresh, nutritious produce directly to food-insecure residents. In partnership with FoodSmart, a leading tele-nutrition company, this initiative addresses the intersection of healthcare and food access, ensuring that underserved communities have consistent access to healthy food. The planning grant will enable sourcing from local, minority-owned farms, fostering economic empowerment for these growers and integrating them into institutional procurement systems. By connecting local farmers to new markets, including healthcare providers and food programs, Dion’s Chicago Dream is helping transform the food value chain, benefiting both communities of color and local producers.
